He was a graduate of Lincoln High School and attended Iowa State University. He was born July 10, 1956, in Des Moines, Iowa, to Eugene and Marie (Harbert) Cook. On July 18, 1981, he married Ruth Ann McMillan in Des Moines. Mr. Cook was a Certified Public Accountant and a member of the Custer Road United Methodist Church.

He is survived by his wife, Ruth Ann Cook, of Plano, Texas; daughters, Melissa and Emily Cook, both of Plano; brother, Larry Cook of Andover, Minnesota; and mother, Marie Cook of Indianola, Iowa.

His father preceded him in death.

A memorial service will be held at 7:00 P.M., Tuesday, January 10, 2006, at Custer Road United Methodist Church with Rev. John Baldwin and Tim Morrison officiating.
